Sports Eligibility

Sports eligibility at Rainier Middle School requires three things,

• Current physical from doctor (paper stamped AND signed by provider)
• $15 ASB fee paid (online or in-person)
• Student registered with Final Forms (upload physical and answer all sports questionnaires)
